Fees & Insurance

• Momentum Speech & Language Therapy of Ann Arbor is a private pay practice at this time.

• Information regarding fees will be discussed during the initial consultation. ​

• A sliding scale may be available on an individual basis. 

 

• Payment is due at the time of service. ​

• Cash and personal checks (with valid ID) made out to Momentum are accepted methods of payment. ​

• If your employer offers a flexible or health spending account (FSA/HSA), you may be able to apply these funds to cover the cost of speech therapy services.

• A superbill can be provided to submit to insurance companies for potential reimbursement of out-of-network services. This would include a detailed receipt containing specific codes (ie, diagnostic and procedural) that insurance companies require in order to process a claim.​
